NOTICE TO DEFENDANTS: (1) JOHN DOE; OR (2) JANE DOE. READ THESE PAPERS CAREFULLY!

You are hereby required to appear and defend the complaint filed against you in the above entitled action within thirty (30) days from the date of first publication of this summons, which occurred on April 19, 2016, and in the case of your failure to do so, for want thereof, plaintiffs will apply to the court for the relief demanded in the complaint. The object of this Complaint and demand for relief are claims for indemnification and contribution for any damages to Michael Cooley and Lori Cooley caused by the negligence of John Doe/Jane Doe related to a hit and run accident which occurred on or about June 15, 2013 in which John Doe/Jane Doe, driving a truck in a negligent manner, struck Michael Cooley, while he was riding his bicycle northbound on North Interstate Avenue near the intersection of North Greeley Avenue in Portland, Multnomah County, Oregon and causing significant injury to Mr. Cooley.

You must “appear” in this case or the other side will win automatically. To “appear” you must file with the court a legal document called a “motion” or “answer.” The “motion” or “answer” (or “reply”) must be given to the court clerk or administrator within 30 days of the date of first publication specified herein along with the required filing fee. It must be in proper form and have proof of service on the plaintiffs’ attorneys.
