REQUEST FOR PROPOSAL: REMOVAL AND REPLACEMENT OF FLAT ROOF
BEND, OR
Proposals due: June 7, 2016 @ 2:00 pm
Sealed Proposals will be accepted by David L. Crowther at the Bend Park & Recreation District Administration Office, 799 SW Columbia Street, Bend, Oregon 97702 until 2:00 pm, June 7, 2016, at which time and place Proposing will be closed, and the Proposals opened and read. No Proposals will be received after closing. Each Proposer is required to submit a First-Tier Subcontractor Disclosure Form, as required by ORS 279C.370, in a separate envelope within two (2) hours of the Proposal closing time. The District will reject a Proposal if the Proposer fails to submit the required disclosure form by this deadline.
The project is located at the Juniper Swim & Fitness Center, 800 NE 6th Street, Bend OR, 97701. The work includes, but is not limited to, removal of existing TPO roof system and selected tapered insulation and installation of new EPDM roofing or similar, modified tapered insulation system and related flashings and roofing details. Work to be performed between the months of June, 2016 to October 2016.
The Proposal (“Proposal”) shall be submitted in a sealed envelope and marked: JSFC North Flat Roof Renovation. Prequalification prior to submission of a Proposal is not required.
Plans, specifications, addenda, and notifications of Proposal results for this project may be viewed, printed or ordered on line from Premier Builders Exchange at http://www.premierbx.com, then click on Public Works Projects. Bidders are responsible for checking this web site for the issuance of any addenda prior to submitting a bid. “Prime” bidders should register with Premier Builders Exchange as a plan holder to bid this project. If the prime bidder/general contractor does not register with the plan center, the general contractor will still be held responsible for all addenda’s/changes to the documents and will be considered non-responsive if their bid does not reflect those addenda/changes.

The successful Proposer must comply with the Oregon Public Contracting Code (ORS 279A, 279B and 279C) and the District’s public contracting rules, as all are amended from time to time, in the performance of the work.
The project is a public works project subject to the state prevailing wage rates under ORS 279C.800 to 279C.870. No bid will be received or considered unless the bid contains a statement that the bidder will comply with ORS 279C.838, 279C.840. The wage rates will be found at http://www.oregon.gov/boli/WHD/PWR/Pages/pwr_state.aspx and January 1, 2016 with April 1, 2016 amended PWR Book. Each bid must be accompanied by a bid security of five percent (5%) of the amount bid.
The District may reject any Proposal not in compliance with all prescribed public Proposing procedures and requirements and may reject for good cause all Proposals upon a finding of the District that it is in the public interest to do so.
No Proposal shall be received or considered unless the Proposer is licensed by the Construction Contractors Board, or by the State Landscape Contractors Board, as applicable, as required by ORS 671.530.
This work does not require a licensed asbestos abatement contractor.
