Notice is hereby given that on April 1, 2016, Mark Cristofolo, Petitioner, filed a Petition in the above-titled court and proceedings to be appointed as your conservator. A true copy of the Petition is attached hereto.

Objections to the conservatorship must be filed in the conservatorship proceeding in the above court on or before April 23, 2016.
Written objections may be made by mailing or delivering the objection to Room 224, 1021 S.W. 4th Ave., Portland, Oregon 97204. Oral objections may be made by appearing in person.
The court may conduct a hearing to consider the conservatorship petition. The judge will review the reports and any evidence presented at a hearing and then has authority to appoint a conservator for you if that appears necessary. If the court appoints a conservator for you, the conservator will take possession of your money and property and will have broad power to manage that money and property; however, the conservator’s control over your money and property must be exercised for your welfare and benefit.
Your financial resources may be used to pay court-approved expenditures for the conservatorship.
In addition to the right of written and oral objection, you have the following rights in the proceeding to have a conservator appointed for you:
(1) The right to be represented by a lawyer, to contact and retain a lawyer at any time, to thereafter contact that lawyer at any time, and to have records released to the lawyer;
(2) The right to be informed of free or low-cost legal and other relevant services available in this area. Examples of organizations that may provide free legal services to you are as follows: Disability Rights Oregon, 620 S.W. Fifth Ave., #500, Portland, Oregon 97224, 503-243-2081;
(3) The right to have access to your personal records;
(4) The right to request a hearing;
(5) The rights to present evidence and to cross-examine witnesses;
(6) The right to request that the power of the conservator, if any, be limited by the court; and
(7) The right to request the removal of the conservator or a modification of a conservatorship order.
