Notice to Adoption

IN THE MONROE CIRCUIT COURT STATE OF INDIANA COUNTY OF MONROE CAUSE NUMBER: 53C0-1711-AD-000117 IN RE THE ADOPTION OF: AVERY DOUGLAS BAKER. A MINOR NOTICE OF ADOPTION Evan Douglas Baker is notified that a petition for adoption of Avery Douglas Baker, born October 16, 2014 to April (Webb) Jette, was filed in the office of the clerk of the Monroe Circuit Court, 301 N College Ave., Bloomington, IN 47404. The petition for adoption alleges that Evan Douglas Baker’s consent to adoption is not required pursuant to IC 31-19-9-8 for failure to support or have contact with his child. If Evan Douglas Baker seeks to contest the adoption of the child, Evan Douglas Baker must file a motion to contest the adoption in accordance with IC 31-19-10-1 in the above named court not later than thirty (30) days after the date of service of this notice. If Evan Douglas Baker does not file a motion to contest the adoption within thirty (30) days after service of this notice the above named court will hear and determine the petition for adoption. The consent to adoption of Evan Douglas Baker will be irrevocably implied and Evan Douglas Baker will lose the right to contest either the adoption of the validity of Evan Douglas Baker’s implied consent to the adoption. No oral statement made to Evan Douglas Baker relieves Evan Douglas Baker of Evan Douglas Baker’s obligations under this notice. This notice complies with IC 31-19-4.5-3 but does not exhaustively set forth a person’s legal obligations under the Indiana adoption statutes.
