REQUEST FOR PROPOSALS: NEIGHBORHOOD INFRASTRUCTURE IMPROVEMENTS PROJECT: GCD1005731 MEDFORD, OR Proposals due: August 31, 2017 @ 2:00 pm Sealed proposals addressed to Cory Crebbin, Public Works Director, City of Medford, Oregon, endorsed Neighborhood Infrastructure Improvements Project: Plum Street and Tennessee Drive will be received at the office of the Public Works Director, 200 S. Ivy Street, Medford, OR 97501, until 2:00:00 p.m. local time on Thursday, August 31, 2017, and thereafter will be opened publicly and read and will then be referred to the City Council. This project is located on Plum Street from Dakota Avenue to Stewart Avenue and Tennessee Drive from DeBarr Avenue to 900 feet North (including a portion of Chico Street) all within the City of Medford and Jackson County, Oregon. This project includes all labor, equipment, and materials necessary to construct approximately 2,100 linear feet of sidewalk. Pay items include, but are not limited to the following: 370 yd general excavation; 330 ton aggregate base; 656 ft curb and gutter; 8,668 ft concrete walks; 2,376 ft concrete driveways; and 45 ton HMAC. Bids must be accompanied by a certified check equivalent to 10% of the proposal payable to the City of Medford, to guarantee that if a proposal is accepted, a contract will be entered into and its performance secured. A Bid Bond to like effect and amount with a corporate surety will be acceptable for this project. Bids must be in writing and signed by or on behalf of the bidders. All Bidders shall be prequalified with the City of Medford, as provided by law under Oregon Revised Statutes, at least 5 days prior to opening of bids. All of the provisions of Section 279C.800 through 279C.870 ORS, as amended by Senate Bill 477, relating to wage rates to be paid on all contracts for Public Works in this state must be complied with and the statement attesting to the contractor’s willing ness to do so must be signed and submitted with the bid. All bidders must state at the appropriate place in the bid documents, whether or not the bidder is a resident as defined in ORS 279A.120. A percentage increase, as determined by the Department of General Services, will be added to nonresident’s bids for the purpose of determining the lowest qualified bidder.
