ADDENDUM TO ARCHITECT/CONSULTANT

GERVAIS SCHOOL DISTRICT ADDENDUM TO ARCHITECT/CONSULTANT Proposals due: June 28, 2021 @ 10:30am REQUEST FOR PROPOSALS This is an addendum to the Gervais School District’s Architect/Consultant Request for Proposals published May 17, 2021 (the RFP) and forms a part of the contract documents and modifies the original scope of work, specifications, requirements and deadlines. Except as changed by this Addendum, all terms, conditions, scope of work, plans, specification, requirements and deadlines in the original RFP and previous addenda remain unchanged. This addendum is intended to clarify the following terms: 1. Deadline for Submissions: The Proposal deadline is hereby extended for one week. Proposals must be received no later than 10:30 a.m. on June 28, 2021. All subsequent RFP deadlines and anticipated dates, occurring on or after the original proposal deadline of June 21, 2021, are also extended for one week as follows: Proposal Closing: June 28, 2021; 10:30 A.M. Evaluation of Proposals: June 29 – July 2, 2021 Interviews: July 6 – 7, 2021 Notice and Selection: July 26, 2021 Contract Negotiations (subject to an extension by the Board): August 2 – August 9, 2021 Design Finalization: December 2021 Construction: August 2022 – August 2024 2. Section 4.00, subsection 2.6 on page 7 of the RFP: The Architect/Consultant, as selected, will not review and provide advise on the Design Team’s documents for completeness, adequate detailing, compliance with program and adherence to codes or applicable agency requirements, but will oversee construction services. 3. Section 4.00, subsection 2.10 on page 7 of the RFP: All other permit fees will be applied for and paid for by the general contractor 4. Section 4.00, subsection 3.5 on page 7 of the RFP: As clarification, the Architect/Consultant will observe the work to verify conformance with bid documents and quality control guidelines. The general contractor will further establish and maintain a Quality Control (QC) program for the construction portion of the Project. 5. Section 4.00, subsection 3.6 on page 7 of the RFP: The RFP stated that the Architect/Consultant will be responsible for some or all of a list of tasks. As clarifications, the general contractor shall establish and implement Drug Testing and Safety Plans in accordance with State Law under 279C.505(2) but Architect/Consultant will oversee construction services and ensure contractor compliance with all laws. 6 Section 4.00, subsection 3.7 on page 7 if the RFP: The Architect/Consultant, as selected, will provide training to the Design Team’s operational and maintenance staff, as needed. 7. Section 7.00, subsection 2 on page 9 if the RFP: As clarification, Architect/Consultant need not be licensed under ORS 468A.720 for asbestos abatement but shall confirm that any contractors or subconsultants be licensed under ORS 468A.720 for asbestos abatement, as required. 8. Section 7.00, subsection 3 on page 9 if the RFP: Prevailing wage rate requirements will apply to all contractors and subcontractors working on Phase 3 of the Project. Published June 23, 2021.
